About Us

The National Shared Care Plan Programme was initially formed by the National Health IT Board to initiate pilot projects which trial approaches to shared care to support long-term condition management. The prgramme is now embedded in the Northern region.

healthAlliance, working with Northern regional District Health Boards and primary / community providers manages the programme.
